The Program Administrator has released the final Community Solar Standard Disclosure Form. Approved Vendors are required to provide a disclosure form to all potential subscribers to approved ILSFA community solar projects and must provide a signed disclosure form as part of the verification of actual subscribers.

Approved Vendors with community solar projects will be provided a fillable PDF of the disclosure form and a supporting community solar savings calculator spreadsheet to use when completing the disclosure form. The Program Administrator is working to make automatically-generated disclosure forms available through the Approved Vendor portal.

The draft Community Solar Standard Disclosure Form was first posted on November 1, 2019, and the public comments and response to the public comments were posted on November 21, 2019 and January 14, 2020, respectively.
